Docker: remove Postgres and Nginx setup from Dockerfile

Both Postgres and Nginx will be set up through Docker compose in the
future. The CATMAID configuration has been moved out of the Dockerfile
as well and lives now in a new entrypoint file.

Also the createuser.sh script has been modified to only create a new
user and a new database if they don't exist already.
