Confirmation status (#248)

* Refactor slots as objects

So that slots can have context around confirmation or default values,
an `alexa.slot` object can encase this logic.

* Add confirmation status for intent and intent slots

To track confirmation of individual slots and of the intent itself,
variables and an introspective function have been added.

* Bump Changelog for confirmationStatus

* Remove now-unnecessary try/catch

Now that we are explicitly checking for the slot availability, there’s
no need for the try/catch block.

* Add README entries for the new confirmationStatus logic
