refactor: playbook uses singleton pattern

Multiple hubot script files would have previously created new and isolated instances of Playbook.
This change allows playboook and its modules to be persistent and referenced acrosss files. It can
still be recreated for tests however.

require/import syntax needs update to get singleton instance - see tests
