bugfix: treat $length_limit as an actual limit

With the previous code, a string that was, say, 21 characters long was
truncated to 20 (if the limit was 20) and then extended to 24 once the "
..." appended. With the new code, it is guaranteed that the final string
will not exceed the given limit.
