Upgrade dependencies

Some lint violations had to be fixed as part of this change due to a
major version bump in Standard style.  All were violations of type
no-unused-expressions related to Chai expect/should assertions.  There
are some pending ESLint issues open to address this in ESLint core by
adding a "chai" environment option.  There are also some ESLint plugins
to target Chai specifically, but I'd prefer the core option if it is
released relatively quickly.
