* Advance to "Support for composite primary keys in schema" commit of 23 Jan 2016.

* Update generation of CREATE TABLE and ALTER TABLE syntax to be more like Postgrex.

* Format tests more consistently.

* Enable test "create table with composite key."

* Make handling of :drop table more like Postgres.

* Make creation of indexes more like Postgrex.

* Make renaming a table more like Postgres.

* Column constraints aren't supported in SQLite.

Throw an error when trying to add them.

* Make handling of drop index more like SQLite.

* A few more tweaks to be more consistent with Postgres.

* Restore remaining CREATE INDEX tests.
