Updates to ensure unique layer names with mapproxy-served layers (#117)

* Update OGRImport.import_file() and utils.import_all_layers() to ensure the
  configured layer name is used over the default.

* Update MapProxyGPKGTilePublishHandler to use configured layer name for cache & grid names
  in mapproxy config yaml.
