Remove obsolete middleware caching setting

The `CACHE_MIDDLEWARE_ANONYMOUS_ONLY` setting is obsolete and ignored
as of Django 1.8, and was not necessarily a good idea prior to that.
See https://code.djangoproject.com/ticket/15201

Removing for these reasons, and because middleware caching is no longer
enabled by default in ICEkit projects anyway.
