Miscellaneous improvements on inputhost

* Make sure the flush ticker is controlled by extHost rather than
  per replica connection, so that we flush to store at the same time.
* Move the flush threshold and flush timeout to a common location which
  can be shared by all services. Moved it to knobs.go which will eventually
  hold all the dynamic config values.

TODO: Add dynamic knobs to input, output and store to tune them
individually..
