Replaced org.kohsuke:libpam4j with personal fork.

This should help improving and bugfixing the main library regardless of whether or not changes are getting merged.
Details:

- using https:// so that anyone can run `git submodule update --init`.
- pull submodules' sources in TravisCI.
- exclude src/main/java/org from JaCoCo and coverage reports.
- improved documentation.
