refactor: clean up parsing, organize modules

We expose more types as public types (QType, Opcode), preferring to use
the std::result::Result type rather than just the nom::IResult type.
I am still on the fence as to whether the nom types need to be exposed
at all by this library, but I'm leaning towards a no. Further error
types need to be defined, built out, and implemented before this will
work properly in error scenarios.

I've also found new ways to use the nom macros, simply by reordering
the arguments where the `data` parameter (containing the entire message
slice) are concerned, and using the `apply` macro. This has simplified
many parsers; there may be more simplifications that can be applied.
