Improvements and fixes to IMAP provider compatibility

Fixes body text and attachment incompatibilies with Exchange, e.g.,
through Office365 or Rackspace hosted email

* Included EmailInbox id in attachment download link to prevent
  attempting to retrieve attachments from incorrect inboxes
* Updated email body parsing to perform case insensitive email
  disposition type detection
* Added line breaks to plain text body. This prevents plain text emails
  from being rendered on a single long line
