Support Coveralls Enterprise

This adds an environment variable for overriding the coveralls host,
which is a useful thing to do for self-hosted Coveralls Enterprise
setups.

To use, set the `COVERALLS_HOST` env var before calling coveralls.
For example:

 COVERALLS_HOST="https://cov.my-company.com" coveralls
