Gracefully handle invalid encodings when parsing

Some invalid encodings would cause a panic when parsing due to the
incompleteness of the decoding table. This commit extends the decoding
table with 0xFF sentinel values for all the remaining possible bytes.

This is tested with TestParseRobustness. Documentation has been changed
to reflect this change.
