Merge branch 'refactor-endpoints' into 'develop'

Refactor endpoints

This MR lays the groundwork for supporting the domains API (and arbitrary many more APIs, if necessary) and also tidies up default endpoints such that they're all kept in the lib package rather than a mixture of lib and cmd/bytemark/util.

Hopefully I haven't made any breaking API changes..

Once merged I'll get on with #85 - adding domain registration support.
