Refactoring to be able to test public APIs

- Created injector scripts for both cli and unit test paths.
- This allows for dependency injection (necessary for testing.
- ./src/index.js is now a facade for ./src/api.js.
- ./src/index curries functions such as #add and #rm so the real
API can be injected as well as the mock for when testing.
- Abstracted all Inquirer prompts into ./src/prompts.js for same
reasons as above.
- Removed ./src/generateEntry.js and simplified #add logic.
- Moved `npm test` logic into ./scripts/test.sh Bash script.
