Unlink the output DB before each conversion

This way, the "file change counter" is always reset, meaning that converting
the same list over and over always results in the same output database.
This makes the databases more stable.
See also https://www.sqlite.org/fileformat.html for more information.

As a consequence of this change, all databases have been converted again to
reset the counter.
