MAINT: fix many small things in MRC reader and related

- add `MRC2014_SPEC` to `__all__`
- in `MRC2014_SPEC`: add `NLABL` and `LABEL` fields, change
  `Char` to `String`
- fix wrong mode 0 in `MRC_MODE_TO_NPY_DTYPE`
- make `set_attrs_from_header` public
- set `extended_header_type` and `text_labels` attributes
  from header
- make `reshape_order` a parameter in `read_data` of the
  parent class and use `'F'` in the MRC reader
- add remaining `struct` type conversions
- move functionality of `reformat_spec` to `spec_fields` and
  rename `spec_fields` to `_fields_from_table`
- rename `standardized_fields` to `header_fields_from_table`\
  and make it work directly on a spec table instead of a
  list of fields
- extend documentation of the spec parser, including a
  definition of the supported table formats
- allow shapes in the data type column of the spec, including
  "incomplete" shapes
- improve error messages
- remove the alternative constructor `from_raw_file`; instead,
  a `dtype` can be given in `__init__` and later on, data
  can be read in arbitrary places using the `dstart` and `dend`
  parameters
- make the most important attributes properties so they have
  a docstring
- add `set_attrs` argument to `read_header`, to avoid KeyError
  when using non-standard fields
- handle string fields properly
- many other small fixes
