Major overhaul: making this module a real abstraction (#18)

* refactor: split internals into multiple files

* feat: become a real abstraction for loading config

* docs: update readme

* tests: temporarily disable error message tests

A module dependency experiences an error on node 7 (see Qix-/node-error-ex#6) when trying to throw a custom Error.
The issue has been reported, and will eventually be addressed. In the meantime, the error message will not be helpful.
