Fix session handling.

@key generation was changing every time the middleware was initialized, causing lost sessions.

Sessions would timeout after expires_after seconds passed unless the session was modified. Now, there is a parameter, update_timeout, and sessions are changed after refreshed if they are at least accessed after that time. This logic only triggers if the session is actually loaded.
