Add MouseButton: named mouse buttons including the horizontal wheel

Widgets compared mouse buttons against bare numbers. MouseButton is an
IntEnum whose values are those numbers, so existing comparisons still hold:

- UNKNOWN = 0 for an X10 release, LEFT, MIDDLE, RIGHT, WHEEL_UP,
  WHEEL_DOWN, WHEEL_LEFT = 6, WHEEL_RIGHT = 7, BACK = 8, FORWARD = 9.
- MouseButton(n) for n above 9 returns an uncached BUTTON_<n> member,
  so terminal reports of unnamed buttons convert without raising and
  without growing the member map.
- urwid's own widgets and the Terminal's mouse report tables use the
  members; screens and testing.click() still pass plain ints.
- The mouse input manual documents buttons 6 to 9, which screens deliver
  which buttons, and when a release names its button.
