Finish the TIME_NS read follow-up: one conversion, TIME path as before

- DuckDBTimeNs.ToDuckDBTimeOnly() is the one TIME_NS to DuckDBTimeOnly
  conversion; DuckDBValue and both reader paths use it.
- The TIME_NS branch of GetValidValue moves into the final switch, so a
  TIME read no longer checks for TIME_NS first.
- Remove the unused NanosecondsPerMicrosecond constant.
- DESIGN.md: rewrap the TIME/TIME_NS/TIMETZ paragraph, say how TIME_NS
  24:00 reads, and that DuckDBTimeOnly drops sub-microsecond digits as
  DuckDBTimestamp does for TIMESTAMP_NS.
- Tests: merge the two overlapping TIME_NS precision tests, read TIME_NS
  in a struct field and a map key and value (with NULLs), as DateTime
  through the object path, and 24:00 through DuckDBValue.
