Add CommandMap help text, categories and key check

CommandMap can now describe each command for a keyboard help screen.

- CommandInfo holds a command's help text and category; every standard Command has one.
- CommandMap.register() describes a command and binds keys to it.
- register() raises ValueError when a key is already bound to another command.
- command_info is a read-only view of the descriptions; keys_for() lists a command's keys.
- copy() copies the descriptions; a map pickled before them gets the defaults.
