3.0217_03: secure types, avoid overflows

expect_false() macro fix for MSVC
add HVMAX_T and RITER_T,
fix av and hv length types: protect from security sensitive overflows
and int wrapping.
add new "Hash key too large" error, do not silently truncate overlarge
hash keys > I32_MAX.
