* docs: make fp-go skills correct and consistent

Fixes:
- lens generator command: go run github.com/IBM/fp-go/v2 lens (there is
  no v2/main package); also in the cookbook
- fp-go-pipe-flow evals now match the skill (no reader for pure code,
  correct ReaderIOResult type, canonical RD alias)
- fp-go Result example: matching error comment, no unused import
- fp-go-mcp: macOS-only Desktop config path, Claude Code setup, raw FTS5
  query strings, accurate get_example matching and temp-file note
- PipeN step counts in fp-go-pipe-flow and fp-go-pr-review

Consistency:
- all front matter uses a folded "description: >-" starting with
  "Use this skill when"; narrower triggers for fp-go and fp-go-pipe-flow
- logging examples use TapIOK / TapSLog instead of ChainFirstIOK
- Option defaults use LZ.Of; net/http is imported unaliased
- pr-review severities agree between rules and the example summary
- shared "Before You Generate" section in every code-writing skill
- Title Case headings, "Common Mistakes", no inline pkg.go.dev links

* docs: clearer required-value accessors for RR and IRR in fp-go-context
