fix: clear a push token FCM reports as unregistered (#31)

FCM answers a dead registration token with 404 NOT_FOUND and an FcmError
detail of UNREGISTERED. That was folded into a plain false, so the dead
token stayed on the device row, the admin list showed the handset as
push-capable, and every later message failed the same way until the
handset next launched.

FcmClient and FcmTransport now return a PushOutcome (Sent, Failed,
Unregistered). The dispatcher clears the token on Unregistered through a
new DeviceRepository::clearPushToken(), which only clears while the row
still holds the dead token, so a fresh token reported mid-send survives.

Detection reads the errorCode, never the HTTP status: a wrong project id
is a 404 too, and clearing every handset's token over it would turn a
configuration fault into a fellowship that has to relaunch Link.
