fix(di): report a circular dependency instead of blocking forever (#236)

A provider graph that refers back to itself deadlocked MakeInjector:
the memoized computation for the first token waited on itself. While a
provider factory runs, the injector handed to it carries the chain of
dependencies above it and returns an error naming the chain when a
token already on it is asked for again; once the factory has returned,
its lookups start a fresh chain, so an injector kept past construction
resolves as before.
