fix: Retrieve tags with git versions older than v2.2.0

Previously, we were using the pretty format '%D' to get the tag name (without extra 'noise'),
but this specifier was added in git v2.2.0 (09/2014) and still not supported in many
distributions and docker images. This fix replaces '%D' with '%d' which has wider support.
