Fix year/e-mail for sending the release to PyPI

- Remove my e-mail from the files, keeping only the __author_email__
  module-level attribute because it's required by setup.py
- Use the e-mail itself instead of escaping it with [at] and [dot],
  as PyPI now requires the author e-mail metadata to be valid
- Remove the "Created at" comments from every Python source (the
  comment before its module-level docstring) and every
  reStructuredText file (the heading comment)
- Change the year range everywhere (mostly copyright messages) to
  be 2012-2016
