More robust handling of spot instance requests

- no longer reinvent the existing waiter functionality when waiting for
spot instances to start.
- defer tagging of the spot instance request until the instance was
found, hopefully the spot instance request is taggable by that time,
which was somehow happening in #20.
- tag the new EC2 spot instance immediately after it was found in
order to remve some tagging calls spread around the code base.
