Add an onUnsubscribe hook

Enable adding some connection state tracking on the server code.

Here are the signatures now:
- onSubscribe(parsedMessage, params, subId)
- onUnsubscribe(graphqlSubId, subId)

By tracking the subId we can now identify if a client is connected or not.
