fix(mcp): address review feedback on native Modern client

Map MCP isError results to AI SDK error outputs, share schema
normalization between discovery and runtime, restore a lightweight tool
shape guard, track session closes by server name, discover enabled
servers in parallel, dedupe in-flight workload fetches, and harden E2E
empty-body bridging.
