* test: add Codex provider auth lifecycle regression matrix (#755)

Walks the full auth lifecycle (login, authenticated call, logout, credential
removal, stale-token cleanup, failed credential, re-add after removal) on the
Codex bridge provider — the regression target — parametrized over OAuth and
API-key auth modes. Unlike the sibling per-scenario suite, this composes the
stages on one instance to pin the cross-stage state-machine transitions that
broke across stale-token and removal/re-add flows.

* test: strengthen auth lifecycle matrix per review

Address review feedback on the composed lifecycle walks and removal
assertions:
- Composed walk re-adds a DISTINCT credential (not the same key/account)
  so the post-readd getModels() forces a genuine second upstream probe
  instead of hitting verifyCredentials()'s 30s probe cache. Asserts the
  second fetch and its authorization header.
- Wrap the two composed refresh spies in try/finally so a failed
  assertion can't leak the mocked global fetch into later tests.
- Assert auth.json is actually unlinked after credential removal (via
  authFileExists) rather than merely unreadable — a truncated/corrupted
  write would have falsely passed the swallowed-parse check.
