fix: remove the refresh button from the default provisioning form (#1122)

manual SSID entry is a sufficient fallback for the dropdown. removing
the button also eliminates the only UI-triggered path that could fire
a scan while a client is associated to the SoftAP -- single-radio
channel-hop drops the phone using the portal. the page-load auto-scan
and the endpoint's ?refresh query param are unchanged.
