Reconcile design docs with the implementation

The docs described generated level structs with ordering methods; what
ships is a builder call chain into rt, and several design decisions had
landed in commits with no ADR recording them. ADR-004 and ADR-010 now
describe the built shape with the superseded design demoted to Rejected
Alternatives, and a new ADR-011 records the constructor stubs that make
generation opt-in per injector and leave naming to the application.

Where the disagreement was fact rather than design, the code won —
notably that an overrun is reported only when a component returns, so a
hung one is never reported at all.

ADRs are rewritten in place rather than amended; docs/adr/README.md
states that rule and the pre-1.0 scope it holds under.
