fix blank pane after hidden split promotion (#304)

* fix(panes): remount promoted split surface

when the primary shell exits while its split is hidden, the survivor moves into the primary slot without changing the surrounding SwiftUI branch. Include the surface instance in the host identity so SwiftUI replaces the torn-down view.
