Add label kwarg to bootstrap_field (#853)

There was no way to override a field's label text at the template
level — only via the Django form/field definition. The workaround
(using bootstrap_label directly) doesn't produce horizontal/floating
layout markup, since that's computed by FieldRenderer.

self.label defaults to field.label and feeds both get_label_html and
default_placeholder (so overriding the label also updates the
autopopulated placeholder, consistent with the existing behavior
where the placeholder already defaults to the label text).

Fixes #635.
