feat(cc608): inject CTA-608 captions in CMAF/LOCMAF/LOC/moq-mi serve paths

Add a -cc608 flag to mlmpub that installs an enabled cc608 caption generator on video tracks. The per-group SEI schedule is computed once per MoQ group and spliced before each frame's first VCL NALU: in createFragment for CMAF/LOCMAF (pre-encryption), and via a shared videoCaptioner helper for the LOC and moq-mi raw-frame loops. A nil/disabled generator, non-video tracks, and non-AVC/HEVC codecs (AV1) are complete no-ops.
