feat: add openapi-meta root primitives (nullable union, open_fragment, root_close) (B1-1059 PR-c) (#1045)

- nullable field-meta union type: bb_serialize_field_meta_t.nullable emits
  a 2-element "type":["<base>","null"] union for a scalar-leaf row instead
  of the bare type value; defaults false (every existing meta table's
  trailing designated-initializer default), byte-identical for all current
  callers
- extract shared bb_oa_write_required() out of bb_oa_write_body()'s inline
  required-array block -- pure refactor, byte-identical output
- add bb_serialize_meta_openapi_open_fragment()/_root_close() for
  reconstructing a composite root's schema out of an open/splice/close
  triple instead of a fixed hand-authored base/suffix string pair
- engine-only, inert this PR -- zero callers, no bb_health production code,
  no wiring
