feat: add refactor code actions

Adds source.organizeImports (sorts import module declarations and drops
ones already flagged unused) and refactor.extract for both extract-to-
variable and extract-to-function, reusing the existing unused-import and
scope-walking logic rather than re-deriving it.

Closes #121
