[-] fix stale VIP state after canceled etcd watch, closes #394 (#399)

A canceled or closed watch channel was silently re-armed without
re-syncing state, so leader changes that happened while the watch
was down were lost — leaving the VIP up on two nodes.

- detect closed watch channel in addition to Canceled/Err
- re-fetch the leader key after re-arming the watch
- use WithRequireLeader so the watch fails fast on quorum loss
- bound get() with a timeout so an etcd outage still reports false
- back off 1s between reconnect attempts
