Add v2 intra-pass telemetry (#179)

* Add v2 intra-pass telemetry

Expose intra-pass observer telemetry for the streaming v2 published-profile 0.7.0 path, including pass progress, pending partition detail, trainer subphase summaries, and suspected-stall indicators derived from retained observer-visible state.

Update the streaming-indexer requirements, design, and validation specs to trace the new v2 telemetry behavior and add focused spec-validation coverage for the v2 observer contract.
