Copy signal handlers before emission (#1187)

Signal callbacks can connect or disconnect handlers while dispatch is iterating
the shared list. Iterate a snapshot so current callbacks are not skipped and
new callbacks wait until the next emission.

Add regression coverage for both mutation directions.
