fix(cli): persist mid-turn kb blocks and warn on malformed, ending silent knowledge loss (#154)

- stop hook now scans the whole turn and persists all non-sentinel kb blocks (mid-turn blocks were silently dropped while turnAlreadyPersisted falsely suppressed the nudge)
- malformed/failed blocks now warn via Response.SystemMessage instead of invisible stderr (Stop + SubagentStop)
- SubagentStop never Blocks (OU-222/OU-254)
- persist-skill sentinel still skips (no double-write)
- UserPromptSubmit nudge reworded to call the kb tool + verify with search
