Commit Message

test: raise PHP SDK coverage to ~88% (#3)

Add edge-case, gRPC-client, and Laravel service-provider tests. Lines 88.28%
(693/785), suite green (173 tests). Remaining gap is the gRPC transport in
AuthzDispatcher (only runs with the grpc PECL extension, absent here) and the
AMQP Consumer loop (needs a live broker); the REST paths are fully covered.
Tests only; no production code changed.
