fix(cli): resolve hook prompt-mention project from registered projects, not filesystem walk (#150)

- candidate list now from backlog.ListProjects (projects table)
- removes findWorkspaceRoot/listWorkspaceProjects .claude-walk (stopped at first .claude, never reached the hub)
- prevents spurious subdirectory matches
- cwd tier + OU-283 hub-suppression unchanged
