Add Vitest tests and CI workflow

Introduce testing infrastructure: add Vitest config and multiple test files (test/**) for env, logger, permissions and verification logic; add a GitHub Actions workflow (.github/workflows/test.yml) to run tests, collect coverage and upload to Coveralls; update README with testing docs and coverage badge; add coverage/ to .gitignore. package.json and lockfile were updated to include test-related dev dependencies and scripts.
