feat: capture boot output after flash_external (BR-30) (#37)

- reuse the existing captureBootOutput helper in handleSerialFlash after
  the managed port restart, matching handleFlash/handleErase/handleReset
- add boot_wait optional param to the flash_external tool schema, default
  2.0s, mirroring esp_flash's default
- return boot_output in the response, omitted when nothing was captured
- add tests: boot output present when the mock session yields boot
  lines, and the field cleanly omitted with boot_wait=0
