docs: rewrite README against PROXY protocol spec 3.4

Spec links point at version 3.4 (2026/04/27). New Security section
quoting the spec's trust requirements with a TrustProxyHeaderFrom
sample, a UDP section covering the per-datagram helpers and the
net.PacketConn wrapper guidance, an examples table, and warnings about
the REQUIRE default throughout.
