docs: add v5 refactoring design analysis and migration note draft

Ergonomics-first design reference for the v5 work tracked in #43: per-item
analysis (Chain/Builder decoupling, typed context, formatter-based Key,
nested models, predictable to_dict) with before/after API, breaking-ness,
effort and migration, plus a sequencing plan and a draft migration note for
the docs.
